Did you layer the hand towards the bottom and make it more ghostly in photoshop? At first I thought it was just another hand, then thought it was a hand, it's reflection, and shadow, and now I'm (mostly) sure it's more of a photoshop effect.

Quite nice: I like how the hand that seems more ghostly has a shadow and the more "real" or solid hand has none.

I use long shutterspeed and flash :) It's not photoshopped.
